Thats not artillery, its just a heavy machine gun. But Im guessing the last time someone did maintenance on the gun, they didnt put something in the right place

Well yeah, a double feed or round malfunction is totally different than a user error, though it can also cause the shattering of the chamber, whereas that gun just up and fell to pieces. The m16 I was issued was infamous for double feeding, but in big (expensive) equipment like this, its usually very rare.
